reason might be 
1) Thepost you hav ebeen tagged has been deleted 
or has been hidden
or the post /page got hidden
which in simple term mean the post got archieved 

2) the channel/page which has the picture has gotten hidden 

3) the notification on your feed section, has gone over loaded and then the post was pushed beyond refresh mode

4)instagram wants their users to be very active on their page
thus therefresh time for the feed and the total number of post on it is 25-30 so that users keep on checking their posts regularly!

5) the person who tagged you have deleted the comment from it or the page moderator has deletd the comment from it s post 

conclusion 
if there is a post which you have been has been missed out
goto you computer open the instagram page from there and then explore your feed section comapletly

Cafe Coffee Day owner VG Siddhartha dead,  A Dead body found 7 REASON WHY  The CCD owner reportedly wrote a letter stating the reasons that drove him to take the extreme step  CCD Owner &  founder V G Siddhartha, who had gone missing from Mangaluru on Monday night, has been found dead by local fishermen in the Nethravathi River.   A local Fisherman saw the body floating and immediately went and got the body to the SeaShore. He then went and report this to the nearby police station.  The police responded immediately and took the body to hospital for postmortem  Commissioner of Police (Mangaluru) Sandeep Patil, who rushed to the spot, directed the police personnel to send the body to government-run Wenlock Hospital to conduct a post-mortem. “The body was found in Hoige Bazaar at 6am. We have also informed the matter to his family. The body has been sent to Wenlock Hospital for taking up formalities. We will continue the investigation,” PTI VG Siddhar
